Evergreen Diabetes Program

The Evergreen Diabetes Program provides patients with the self-management skills needed to live with diabetes.

The Diabetes Program serves around 1,000 people a year. About 90 percent of them are able to keep their blood sugars in a good range six months after completing the Basic Skills series, which gives them a very good chance of avoiding long term complications from diabetes. Diabetes Basic Skills Series
The four-part Diabetes Basic Skills series provides patients with a clear understanding of diabetes and the ability to know whether or not their diabetes is under good control. They also learn how to eat for diabetes, how to identify and resolve problem blood sugar patterns, what preventive tests need to be done, and when to call the doctor. The series is taught by a Certified Diabetes Educator.
 
Individual Consultations
Individual consultations are available for those who are unable to do the series, but the series is recommended because the length of the class and the interaction of class members is very conducive  to behavior changes.

Insurance Coverage
Most insurances will pay for the Basic Skills series, but you should check with your insurance company to see what your individual benefits are. Some scholarships are available for those who might not be able to afford the class.
